What's in Your Toothpaste? A Dentist's Guide to the Label

Most toothpaste questions come down to one distinction: mined versus manufactured. Clay, chalk (calcium carbonate), hydroxyapatite, mica, and titanium dioxide are dug out of the ground and carry whatever the ground carried. Hydrated silica and sodium fluoride are synthesized, and test far cleaner. That single fact predicts nearly every surprising result in independent toothpaste testing — and it means "natural" often points the wrong way.

I'm a general dentist in Johnson County, and patients ask me about toothpaste more than any other product. For years the honest answer was "the brand matters less than you think." That changed in 2025, when an independent community-funded testing initiative (Lead Safe Mama LLC) had an accredited third-party laboratory test 51 toothpastes and tooth powders for heavy metals. The results reorganized how I answer the question — not because most toothpaste is dangerous, but because the products people assume are safest were, on average, the most contaminated.

This guide walks the whole label: abrasives, actives, whiteners, and the marketing words that mean less than they sound like. Every number here comes from that 2025 dataset, from manufacturer technical documentation, or from a named, published study — and where something hasn't been independently tested, I say so.

What did independent testing actually find in toothpaste?

Of 51 products tested in 2025: 90% contained detectable lead, 65% arsenic, 47% mercury, and 35% cadmium (parts per billion). Five products were non-detect on all four metals — and they were ordinary-looking tubes, not premium "clean" brands: Dr. Brown's Baby Toothpaste, Kids' Spry Tooth Gel with Xylitol, Pegciz Foam, Orajel Training Toothpaste, and Miessence Mint.

Two context points belong next to every figure in this guide. First, toothpaste is spat out, not swallowed — the absorbed dose is a small fraction of the measured concentration, and most products did not exceed federal limits. Second, the testing methodology has drawn criticism from industry bodies. Reported findings, attributed to their source, are what you'll get here — not accusations.

Why does "natural" toothpaste test worse?

Contamination enters through one door: earth-derived minerals. The dataset included a decisive experiment — two raw ingredients tested alone, with no formulation around them. Single-ingredient calcium carbonate came back at 292 ppb lead and 274 ppb arsenic. Single-ingredient hydroxyapatite came back at 269 ppb lead and 59 ppb arsenic. The ingredient, not the manufacturer, is the source.

Removing fluoride means replacing a synthesized active with mined minerals — and organic certification does not include heavy-metal testing. That's the quotable version of the whole problem. The marketing sells the absence of one ingredient without disclosing what came in with the replacement.

What do the abrasives in toothpaste do?

Every paste needs an abrasive to remove plaque and stain. Abrasiveness is measured as RDA (relative dentin abrasivity) — and here's the part most people never hear: on healthy, intact enamel, abrasivity barely matters. An in situ study found no significant abrasion effect on sound enamel — acid erosion was what moved the needle. The moment gums recede and root surfaces show, that flips: roots have no enamel, and RDA becomes the governing variable.

So the right question isn't "what's the gentlest paste?" — it's "are my roots exposed?" If they are, look for a verified RDA of 70 or below. If they aren't, almost any mainstream paste is fine, and whitening formulas are the main category worth skipping (they're the most abrasive group on the shelf, and where titanium dioxide lives). Fluoride, hydroxyapatite, and the actives question

Sodium fluoride remains the best-evidenced cavity-prevention ingredient available, and for people who keep getting cavities, prescription-strength 5,000 ppm fluoride is the single most effective product change we can make — ask us about it at your next visit.

If you'd rather not use fluoride, there is now one alternative with real trial evidence: hydroxyapatite — the same mineral your enamel is made of. In an 18-month randomized trial of 189 adults, 89.3% of the hydroxyapatite group had no increase in decayed or filled surfaces versus 87.4% with 1,450 ppm fluoride. What about mouthwash?

Here's the counterintuitive one: most healthy mouths don't need mouthwash at all — and daily antiseptic rinses have a measurable downside. Antiseptics suppress the tongue bacteria your body uses to help regulate blood pressure; small but real systolic rises have been measured in short trials. Three free habits that outperform any product

1. Spit, don't rinse. Rinsing with water after brushing washes away the fluoride you just paid for. 2. Never brush right after acid — soda, juice, wine, citrus, sparkling water, or being sick. Acid softens enamel for about 30 minutes; rinse with plain water and wait. What this means for you

Match the paste to your mouth, not to the marketing. If your gums have receded, abrasiveness is your number; if you get cavities, fluoride strength is; if your mouth is dry, flavor and SLS matter as much as the active. Is toothpaste with lead in it dangerous?

If your gums have receded, skip the category.
